- Must be a citizen of Sri Lanka by descent or registration.
- Must be 16 years of age or older.
- Must not already hold a valid National Identity Card.

Birth certificate
Original issued by the Registrar General.
Parents' NICs
Photocopies of both parents' identity cards.
Proof of address
Recent utility bill or Grama Niladhari certificate.
Photograph
Studio acknowledgement receipt (35×45mm, recent).
