- Must have previously been issued a National Identity Card.
- Must declare the loss or damage of the original card.

Declaration of loss
Statement describing how the card was lost or damaged.
Birth certificate
Original issued by the Registrar General.
Photograph
Studio acknowledgement receipt (35×45mm, recent).
